Proof of death certificates:

In Ontario, the Funeral Home chosen to provide services will issue will provide proof of death certificates. The Hodges Funeral provides an unlimited amount. If an Ontario Certified Death Certificate is required (some situations require them as they include cause of death information), click here for the Government Link.

Applying for the Canadian Pension Survivor's Death Benefit

The survivor's pension can be applied for by a person who is legally married or in a common law relationship with a person who has died. May also apply to children of the deceased who are under the age of 18 or between the ages of 18 and 25 attending school, college or university full-time.

The survivor`s pension application requires a copy of the marriage license. If you are unable to find your marriage license, it can be ordered online: Click here for the provincial link. For common law spouses, a form called the Statutory Declaration of Common-law Union must accompany the application. This form must be signed by a Commisioner of Oaths. Canceling an Ontario Health Card and driver’s license

To cancel a family member’s health card following their death, you can visit a Service Ontario Centre

  • Print the Change of information form, complete sections A & F
  • Return the Ontario Health card along with a Funeral Director’s Proof of Death Certificate

Canceling a Canadian Passport:

If the passport is valid, you should mail it to:

Include a copy of the death certificate and a letter telling the government to cancel the passport of the deceased person

You don’t need to return an expired passport. However, if you want the government to securely destroy it, send a note and a copy of the death certificate along with the expired passport asking for its destruction.
